Postmortem: Lowtide On-Sale Checkout Outage

Authors: Dani Okonkwo (incident commander), Priyanka Velasquez (reservation service)

Review held: May 20, 2026 · Format: blameless. We examine systems and decisions, not individuals.

Summary

At 10:00 PT on May 16, tickets for the Lowtide Music Festival went on sale to 38,400 fans in our virtual waiting room. Within two minutes, the reservation service began refusing database connections, and checkout failed for most buyers until 11:19. The cause was a mismatch between per-pod connection pool sizes and the database connection limit, which the autoscaler exposed by adding pods faster than we had ever tested.

Impact

• 38,400 fans queued; 11,250 checkout attempts failed with a 500 or timeout.

• 1,870 seat holds were created and never released, temporarily hiding inventory.

• 143 customers were charged without receiving a ticket. All were fulfilled or refunded by 18:00 PT the same day.

The prompt

Create a Doc blameless incident postmortem for the checkout outage during the Lowtide Music Festival ticket on-sale on May 16, 2026 affecting the Halyard Tickets reservation service, checkout API and virtual waiting room, which impacted about 38,400 fans in the queue. Include a summary, impact, a timeline table, root cause, contributing factors, what went well, what went poorly, and action items with owners and due dates.

Make it yours

  • Change the details in the prompt to fit engineers.
  • For example, change it so that the incident was a failed database migration at a grocery delivery app that delayed 9,000 orders and the audience is non-technical executives, so the root cause section avoids jargon.
  • Ask for edits in plain words: tone, colors, length, layout or data.
  • Share the result by link or export it when you are done.
